This grant is available to help with living with a disability that is age related or as a result of illness and may be available to fund part or the costs of, adaptations to home facilities and the purchase of equipment.
Applications for grants may be considered by the trustees for:
- Adaptations to improve access and facilities within your home such as:
- Stair lifts
- Wheelchair ramps
- Bathroom modifications
- Equipment to help at home such as:
- Specialised beds and mattresses
- Bath lifts
- Hoists
- Riser recliner chairs
- Communications equipment
- Mobility equipment
- Vehicle adaptations
- Mobility Scooters
- Wheelchairs
- Specialised Computer equipment and/or software
- Equipment and aids to support those with visual and/or hearing impairment
Grants are not available to pay care costs or to meet any shortfall in care costs for care at home or residential care.
Adaptations and Equipment
Local authorities have a statutory responsibility to help people with an illness or disability to live independently at home. Before approaching BTPF, you will need to have contacted your local authority's social services department for a care needs assessment. The assessment will establish what support, including any mobility aids and/or adaptations you need. If the assessment identifies you as needing support, a care plan will be agreed with you. Once a care and support plan has been outlined, there will be a financial means test. This will determine the amount of state support, if any, you will be awarded.
Equipment required on medical grounds
If equipment is required on medical grounds then your local social services department or the NHS Trust has a duty to provide any equipment which they consider essential to meet your care needs, we will ask you to ensure they are providing everything they should.
Mobility Scooters and wheelchairs
If you are in receipt of the higher rate of the mobility component of Disability Living Allowance, the enhanced rate of the mobility component of Personal Independence Payments, you may be able to obtain an electrically powered vehicle or wheelchair through the Motobility scheme. This option should be explored before approaching the Baily Thomas Provident Fund.
If you require assistance with an electric wheelchair you may be able to receive assistance through the NHS wheelchair service.
Guidance notes for applicants
Please read the following information carefully before you proceed with an application enquiry.
If the application is for the spouse/partner of the former employee of Mansfield Brewery the marriage certificate and a current utility bill (or other official document), evidencing that they live at the same address as the former employee, should be sent with the completed application form.
If the application is for a dependent child of the former employee of Mansfield Brewery, the Long Form birth certificate (evidencing both parents) should be sent with the completed application form.
To apply for a grant, you will be required to provide the following information with the completed application form:
- Evidence that Social Services or the NHS have carried out a care needs assessment.
- A copy of the care plan produced after the care needs assessment has been carried out.
- For adaptations, you will need to provide evidence of the means test having been conducted and the outcome of this as provided by your local authority.
- Details of any grant contribution offered to you by social services and the cost of any additional items not covered by the grant.
